In the United States, hyaluronic acid (HA) viscosupplementation is approved only for osteoarthritis of the knee. However, many surgeons also use the injections off-label in indications such as ankle, shoulder and elbow. If market players were to acquire approval in other joints, it would greatly expand the hyaluronic acid market.
Continuous growth in unit sales drive this market, but there is considerable pricing pressure on these products in many regions.
Osteoarthritis, which is a condition marked by the degeneration of bone joints and cartilage, affected approximately 40 million people in the United States in 2016. Patients who suffer from osteoarthritis experience acute joint pain, tenderness, stiffness, creaking, locking of the joints and inflammation. These symptoms are caused by the degeneration of joint cartilage, subchondral bone and synovial fluid, specifically the polysaccharide hyaluronate.
Hyaluronic acid viscosupplementation has been available in the U.S. as a treatment for osteoarthritis since 1997. The injected hyaluronic acid is rapidly removed from the body, lasting no longer than a week. The injection results in physiological changes that lead to a variety of outcomes, such as endogenous hyaluronic acid production, reduced inflammation response due to effects on cytokines, reduced cartilage and cell degradation, etc. Hyaluronic acid injection also allows for long-term improvements in mobility and range of motion, and prevents further joint and cartilage deterioration.
Overall, the European HA market is stable in terms of market value growth. This has been caused by lower ASPs across many European countries in recent years, due to healthcare budget constraints that have led to lower reimbursement rates. This is especially evident in the five-injection market in Germany.
Across other Asia-Pacific regions, the trends vary dramatically. South Korea is seeing recovery since market declines around 2014, and India is seeing its small market grow rapidly, largely due to Single-Injection, a stark contrast to China’s growth in primarily the Five-Injection market.

DePuy Synthes, for example, is the market leader in the United States, however, does not have a position in the European market for hyaluronic acid. Instead Fidia Farmaceutici S.p.A. dominates the European market, with nearly one-third of the market share, particularly due to their strong position in their home country, Italy. Fidia’s well-established and reputable products, Hyalubrix® and Hyalgan®, maintained a strong presence in these regions.
The single-injection MONOVISC® and the three-injection ORTHOVISC® are marketed and distributed by DePuy Synthes through a partnership agreement with the manufacturer, Anika Therapeutics, which simply collects royalties.
