Laparoscopic access devices are used for puncturing the body to allow a laparoscope, as well as other instruments, to be inserted into the abdominal cavity. Trocars are a particular type of access device that consists of three primary components, namely the cannula, the seal and the obturator. The cannula is a tube-shape shaft which is placed in the patient to allow access into the abdominal cavity during a laparoscopic procedure. The seal is located at the top of the cannula, and allows instruments to pass through the cannula while preventing air from escaping the abdominal cavity, thereby maintaining proper air pressure during the procedure. The obturator is the mechanism that allows the cannula to penetrate the abdomen, and together, these three components make up the trocar. Although veress needles, insufflator tubing, hand-assisted devices, and other components are also considered part of the access device market by some, this chapter addresses only trocars.
